Free Eye Exams For Working Poor In Most States

VISION USA provides free eye exams to eligible, low-income working families. Services are donated by volunteer optometrists who are members of the American Optometric Association and may be limited in some areas.

COMPLETE THIS APPLICATION FORM ONLY IF:

1. Someone in the household is working at least part time;
2. The person seeking care has no public or private insurance that covers eye exams;
3. The person has not had an eye exam in the last 2 years;
4. The household is low-income and unable to pay for eye exams.

Not available in Arizona, California, Hawaii, Kansas, Kentucky, Minnesota, Montana, North Dakota, Wisconsin and Wyoming. In these states, except for Colorado, you may be eligible with a referral. Download and print an application worksheet and deliver completed worksheet along with income verification documents to a charitable organization, social worker, case worker, community health agency for eligibility verification and application submission:

All applications will be reviewed. If an applicant is approved, the agency will receive an email notification of doctor assignment on behalf of the client. It is the agency’s responsibility to provide the doctor information to their client. If you are not currently working with a case worker, see “Referral Agencies.”
