Here’s how to receive your cash back rebate (up to $7.00 before taxes):

1) PURCHASE one (1) 8 oz. tub Pub Cheese® by Pr?sident® Cheddar & Port Wine flavor.

2) COLLECT the original itemized store-identified cash register receipt(s) dated between 08/01/13 and 10/31/13, and the proof-of-purchase (UPC Symbol). Circle the item description and the purchase price located on the cash receipt. The UPC should be carefully removed from each product once the product package is empty.

3) PRINT AND COMPLETE the official mail-in rebate form, including your name and complete mailing address (city, state, ZIP code), daytime phone number, and a brief description (15 words or less) of why you are dissatisfied with the product.

4) MAIL ALL OF THE ABOVE cash register receipt, proof-of-purchase symbol (UPC Symbol), and official mail-in rebate form, postmarked by 11/14/13, to: Pr?sident® Pub Cheese® Guarantee Offer, PO Box 25723, Rochester, NY 14625.

5) TERMS & CONDITIONS. Products must be purchased between 08/01/2013 and 10/31/2013. Offer requests must be postmarked by 11/14/2013 and received by 11/21/2013. Allow 6-8 weeks for verification and delivery of rebate by mail. Only original rebate forms, valid cash register receipt and proof-of-purchase of the eligible package sizes and brands will be accepted; no photocopies allowed and no other package sizes or brands will be accepted.
